Please download and print the below physical and consent forms and bring to your first visit, along with your youth's immunization records. Please note that the physical form must be completed by a medical professional such as a physician or nurse practitioner within the period of 12months prior to, or within last 30 days.  

You must also have the child's birth certificate and/or proof of guardianship.
